Likely causes
- Open phase in incoming 3-phase supply
- Loose/burnt input terminal or input fuse
- Failed input sensing in PSM (rare)
Triage note: Check supply voltage on all three phases and input wiring tightness first. Mirrors as SP9004 / SV0607.
